furniture protection plan is not available after furniture is delivered
to your home. Upon delivery, you will receive a warranty packet
for the furniture protection plan, which will provide you with

THE FOLLOWING BEFORE DOWNLOADING THE SERVICE REQUEST FORM:
If you are attempting to file a Service Request regarding a human
or pet bodily fluid stain, we ask that you initiate your request
by contacting Guardsman at 800.253.3957 or 877.584.4653. If you
are filing a Service Request for any other type of stain or damage,
you may proceed with the Service Request Form below.
In order to submit a request for service, please complete the
Guardsman® Service Request Information Form below. The Service
Request (.pdf) file contains 2 pages and you must answer ALL questions,
enclose a COPY OF THE ORIGINAL SALES RECEIPT and return the completed
form within 10 days. Full completion of this form is required to
process your service request. Only one Service Request Form is
to be used per piece of furniture. You may attach a separate sheet
for additional information or photographs if necessary. Mailing
instructions are given on page 2 of the form. You will be contacted within 10 business days from
time of RECEIPT OF THIS FORM in our office. You will be called
at the phone number you provide on the form. Service Request Form In order to view and print
the form, Adobe® Acrobat Reader™ is required. If you
do not have Acrobat Reader installed on your computer, please click
on the link below, and follow the instructions provided. Acrobat
